Abdominal wall hernias in patients on continuous ambulatory peritoneal dialysis

Abstract

Continuous ambulatory peritoneal dialysis (CAPD) catheters are indicated for patients with end-stage renal failure who require dialysis and who cannot undergo conventional hemodialysis. Peritoneal dialysis (PD) uses the catheter to infuse the dialysis product and to remove excess solutes and fluids from the body. One of the complications of PD is hernia of the abdominal wall, which occurs due to the increase in intra-abdominal pressure caused by the dialysate and the deterioration of the tissue of the abdominal wall. The procedure of choice for hernioplasties is the tension-free mesh, as it reduces post-surgical morbidity and guarantees quality of life for individuals with end-stage renal failure. Thus, the aim of the study is to discuss abdominal wall hernias in patients with CAPD. This is an integrative literature review, which used the PubMed, SciELO and Cochrane Library platforms as databases for the selection of scientific articles, in English. According to the literature analyzed, it is concluded that patients with renal failure using CAPD are prone to develop abdominal wall hernias due to an increase in intra-abdominal pressure due to the dialysate and the deterioration of the abdominal wall tissue, being possible worsening of renal function. The most appropriate surgical technique was Lichtenstein's, although there are disagreements in the literature, since lateral coupling robotically assisted by totally extraperitoneal repair (eTEP), at least for umbilical hernioplasty, was considered to have the best prognosis.
