Lichen planus mucocutaneos differential diagnossis obtained by lesion evolution: case report

Oral lichen planus (OLP) is a chronic inflammatory process mediated by T cells that affects the oral mucosa or skin, with a higher incidence in adult women, with a low global prevalence of around 1.01%. Usually asymptomatic, it has a diverse clinical appearance, and may present in the form of blisters, plaques, papules, atrophic, erosive or reticular. In some cases, it may be symptomatic, with a burning sensation and discomfort. For an assertive diagnosis of OLP, it is necessary to perform a biopsy, with histopathological aspects characterized by hyperkeratosis, absent or hyperplastic epithelial ridges, hydropic degeneration of the basal layer, presence of hyaline bodies and chronic inflammatory infiltrate in the basal epithelial layer and adjacent layers, in some cases, differential diagnosis of other white lesions of the oral cavity is necessary. The objective of the present study is to report a mucocutaneous clinical case of Lichen planus affecting a male patient, leukoderma, male, 58 years old, where the management of the treatment and the evolution of the lesion were decisive to establish the differential diagnosis of lichen planus from leukoplakia. The authors conclude that the differential diagnosis of white lesions of the oral cavity is not always easy, requiring professionals to have a deep knowledge of the pathologies and the discernment to make the differential diagnosis of lesions with similarities, in order to properly diagnose and treat the lesions.
