Abstract

The term Diabetes Mellitus refers to a group of metabolic disorders characterized by hyperglycemia. It may occur due to a lack of insulin or the inability of this peptide to exert its effects. The evolution of a patient after the implementation of the Pharmaceutical Care service in the city of Alvinópolis-MG caught the attention of the Pharmacy course student, who expressed interest in reporting the data with the objective of disseminating in the scientific community how the Pharmacist's performance can be positive in the quality of life of patients with chronic diseases. Along with the internship supervisors, a study group was structured in order to better understand the methods and tools used in the Pharmaceutical Care service. The present work was elaborated in case study format and used qualitative and quantitative data collected from real events. Before submission, the case report was submitted and approved by the research ethics committee. Reporting the results of this follow-up can contribute to the empowerment of other pharmacists, as well as to the appreciation of the role of this professional in the multidisciplinary health team.
