Pott's Disease associated with paraplegia: A case report of osteoarticular tuberculosis

Pott's disease is the term used to define infection of the spine by Mycobacterium tuberculosis. Extrapulmonary involvement occurs through lymphohematogenous dissemination of bacilli. The development of the abscess compresses the spinal cord causing low back pain, paraparesthesias, paraparesis, difficulty in walking. The aim of this study was to describe a case of osteoarticular tuberculosis, addressing its clinical presentation, diagnosis and treatment. B.F.F, female, 43 years old, was admitted to a public referral hospital in São Luís with a clinical picture of unspecified encephalomyelitis, paraplegia, infectious spondylodiscitis with pathological fracture due to collection in the thoracic spine (T5/T6), where it was observed empyema with spinal cord compression (ICD-10: G952) after MRI and CT scan. The patient was confined to bed and had severe functional disability. The clinical picture of the current disease started 3 months ago, with severe chronic neuropathic pain. She was monitored daily by the hospital's physiotherapy team for functional rehabilitation and the nursing team for general care. A posteriori, there was drainage of one of the infectious foci, which was biopsied for histopathological examination, confirming the diagnosis of Pott's disease. He currently uses rifampicin, isoniazid, amitriptyline, baclofen, gabapentin, clonazepam and omeprazole on a daily basis. The patient continues with deep, cutaneous-abdominal areflexia, grade 0 strength and loss of sensation in the lower limbs. Sometimes the diagnosis of Pott's Disease is delayed due to the non-specificity of symptoms confused with lumbosciatic pain. Its real identification occurs when there is already some bone deformity or nerve injury.
