Basic periodontal treatment in a patient with amyotrophic lateral sclerosis in a hospital setting: case reporting

Abstract

Amyotrophic Lateral Sclerosis (ALS) is a progressive neurodegenerative disease of upper and lower motor neurons without affecting intellectual and cognitive activity. We report the case of a 43-year-old female patient, diagnosed with ALS for about 4 years, who required hospital care in the Intensive Care Unit - ICU at the Regional Public Hospital of Araguaia, with dyspnea. and bloody secretion by tracheostomy and afebrile. The blood count showed a decrease in the important figured elements of the blood, such as red blood cells, hemoglobin, hematocrit and leukocytes. Bacteriological culture of tracheal secretion was positive for Staphylococcus Aureus. Clinical examination showed large accumulation of visible plaque and tartar. On periodontal examination: Probing Depth score of zero (1 site with pocket >= 5 mm). Type 1 gingival recession (no loss of proximal attachment). No mobility or furcation injury. When assessing the Bleeding Index on probing score 2 (10% of sites). The procedure performed was basic periodontal therapy, starting with asepsis with 0.12% Chlorhexidine Digluconate, and supragingival scaling. Then, mechanical prophylaxis performed in two steps: robson brush with pumice stone and water; rubber cup and prophylactic paste. Finishing with application of topical fluoride and hydration of the lips with bepantol. Therefore, it is understood that these patients need a multidisciplinary work, including the dental surgeon in a hospital environment and a team that provides homecare dental care, preventing the onset and evolution of fungal diseases (oral candidiasis), VAP and periodontal disease.
