Evaluation of the clinical-epidemiological profile of deaths by COVID-19 in adult men and women in the state of Maranhão

Abstract

The severe acute respiratory syndrome coronavirus (SARS-CoV-2) has spread worldwide and caused Coronavirus Disease 2019 (COVID-19) in approximately 46 million people by November 2020, configuring a pandemic after an alert from the World Health Organization. The most prevalent comorbidities of patients infected with SARS-CoV-2 are systemic arterial hypertension and diabetes mellitus. Such patients who frequently undergo treatment with angiotensin-converting enzyme 2 inhibitors or angiotensin II type I receptor blockers have a considerable increase in the expression of angiotensin-converting enzyme 2 (ACE2) receptors, facilitating the entry of the coronavirus. These same patients end up developing a worse prognosis of the pathology, which can be lethal. Other studies have shown that adult women and men respond differently to SARS-CoV-2 infection. The expression of ACE2 is influenced by sex hormones, thus, it would be one of the reasons for the higher male prevalence. In Maranhão, cases of infection were initially investigated at the end of February, with two suspects. Regarding mortality, males had a higher rate (62%), which shows the relevance of gender in the prognosis. The present research intends to analyze, through a data collection, the clinical-epidemiological profile of deaths by COVID-19 in adult men and women in the state of Maranhão, establishing a comparison between the symptoms of COVID-19, considering not only the epidemiological profile, as well as the sociodemographic profile and associated morbidities, in addition to discussing the incidence of forms of severity of the disease in both sexes.
